Facilitators and obstacles for the evening family commensality

Within the literature, a conflict between an individualistic tendency and materialized cultural values on traditional beliefs about commensality’s value is identified. The present study is based on the use of transversal quantitative methods to approach the influential facilitation and obstruction factors in family commensality occurrence, materialized at the family-shared evening meal, which emerges as the battle flag to maintain this instance. Main results reveal the prominence of those factors related to the individual’s routine in relation to facilitate or obstruct evening commensality performance, in detriment of the related variables to motivations. These findings highlight the relevance to deeply analyze those factors to understand the composition of households or social class as  commensality’s main determinants, and the content of their relationship.

Parenthood practices in cross-border contexts, the case of Guatemalan men in Chiapas, Mexico

Studies of paternity usually emphasize and refer to be provider as one of the main dimensions that male heads of household participate and while little or no participation in other dimensions such as care or playful exchanges with childrens. This work aims to analyze the experience of fatherhood of some Guatemalan men who work on a daily basis in Chiapas, Mexico, either as commuters, temporary-permanent or seasonal workers. It is considered that working life, for men and their families, configures other dimensions of everyday life, such as family life, including the organization and division of domestic work, care and remunerated work, among its members, as well as ways of life together between parent-child and marital relationship. The transborder context becomes a relevant variable for molding the family transborder experiences of male respondents, highlighting the socio-economic situation of families, employment and population mobility possibilities between the two sides of the Guatemala-Mexico border.

El análisis de costos en los hospitales es un tema que ha suscitado un gran interés en los últimos años. Las condiciones del entorno económico y social han impulsado importantes cambios en la organización y gestión hospitalaria, siendo su objetivo prioritario en la actualidad la búsqueda de la eficiencia. El crecimiento del gasto sanitario público y la demanda generalizada de las instituciones sanitarias los coloca lejos de prestar sus servicios con la eficiencia deseable por lo que se exige un cambio en la concepción de la información de costos.

El presente artículo de revisión expone aspectos fundamentales de la estructura de los costos unitarios hospitalarios para el desarrollo de sistemas de información de costos y gestión hospitalaria, siendo su objetivo prioritario la búsqueda de la optimización de los recursos hospitalarios desde un punto de vista económico-social. Se realizó un análisis teóricobibliográfico de los sistemas análisis de costos hospitalarios, topdown y bottom-up, útiles en la asignación de los costos hospitalarios totales a cada servicio clínico. El análisis económico en atención en salud se divide en: análisis de minimización de costos, análisis de costo-beneficio, análisis costo-efectividad y análisis costo-utilidad. Se concluye sobre la necesidad de fortalecer el cálculo, desglose y sistemas de información de costos para su adecuada implementación, asegurando la confiabilidad, pertinencia y oportunidad de los resultados generados por dichos sistemas, tipos y análisis de costos para su uso efectivo para la gestión hospitalaria.

Prolactin (PRL) is a 23-kDa protein that can be synthesized and secreted by pituitary and extrapituitary tissues such as immune cells due to its expression being regulated by two independent promoter regions. The promoter which is responsible for extrapituitary expression contains the single nucleotide polymorphism (SNP) ¿1149 G/T previously associated with autoimmune diseases in various populations. This study evaluates the relationship of PRL ¿1149 G/T polymorphism with PRL serum levels and clinical characteristics in systemic lupus erythematosus (SLE) patients from western Mexico.

Genealogía e historia familiar en la enseñanza de la historia en secundaria

En la actualidad la enseñanza de historia en la educación secundaria se encuentra dividida en dos partes que llevan el nombre de historia I e historia II, la primera se lleva en segundo grado y corresponde a la historia universal, la segunda, historia de México se cursa durante tercer grado, en los nuevos programas de estudios se ha buscado romper con una concepción tradicional de la enseñanza memorística por un modelo basado en competencias y aprendizajes esperados, los cuales se han convertido en una guía didáctica para el docente de educación básica. Estas reformas requieren un cambio conjunto de todas las personas involucradas en la educación, ya sea docentes, alumnos, padres de familia y directivos.

Physicians and pharmacists: collaboration to improve the quality of prescriptions in primary care in Mexico

Background Inappropriate prescription is a relevant problem in primary health care settings in Mexico, with potentially harmful consequences for patients. Objective To evaluate the effectiveness of incorporating a pharmacist into primary care health team to reduce prescription errors for patients with diabetes and/or hypertension. Setting One Family Medicine Clinic from the Mexican Institute of Social Security in Mexico City. Method A “pharmacotherapy intervention” provided by pharmacists through a quasi experimental (before–after) design was carried out. Physicians who allowed access to their diabetes and/or hypertensive patients’ medical records and prescriptions were included in the study. Prescription errors were classified as “filling”, “clinical” or “both”. Descriptive analysis, identification of potential drug–drug interactions (pD–DI), and comparison of the proportion of patients with prescriptions with errors detected “before” and “after” intervention were performed. Main outcome measure Decrease in the proportion of patients who received prescriptions with errors after the intervention. Results Pharmacists detected at least one type of error in 79 out of 160 patients. Errors were “clinical”, “both” and “filling” in 47, 21 and 11 of these patient’s prescriptions respectively. Predominant errors were, in the subgroup of patient’s prescriptions with “clinical” errors, pD–DI; in the subgroup of “both” errors, lack of information on dosing interval and pD–DI; and in the “filling” subgroup, lack of information on dosing interval. The pD–DI caused 50 % of the errors detected, from which 19 % were of major severity. The impact of the correction of errors post-intervention was observed in 19 % of patients who had erroneous prescriptions before the intervention of the pharmacist (49.3–30.3 %, p < 0.05). Conclusion The impact of the intervention was relevant from a clinical point of view for the public health services in Mexico. The implementation of early warning systems of the most widely prescribed drugs is an alternative for reducing prescription errors and consequently the risks they may cause.

Women: Between autonomy and family life

Job training is an important tool in actual days for women, in order to be in better terms to enter or stay in the labor market; however, they suffer different experiences to combine job training and family life. This paper presents a qualitative research based on the biographical method, done with the purpose of analyzing the significance of job training in their personal development, family and working life. The results prove that the increase in the formation of women, has not substantially changed the traditional view about social roles of women and men, but promote their personal and economic autonomy, without sacrificing their reproductive and family responsabilities. Women give values to both domestically and public space, and they consider them as different areas that need to reconcile.
