Sarcopenia Is Associated With Physical and Mental Components of Health-Related Quality of Life in Older Adults

Objectives: To estimate the association between sarcopenia and the health-related quality of life (HRQoL) among community-dwelling older adults.

Design: Cross-sectional analysis of a prospective cohort.

Setting: The Rural Frailty Study, a prospective study on the prevalence of frailty in rural settings in Mexico, with baseline and follow-up measurements conducted in 2009 and 2013, respectively.

Participants: Five hundred forty-three men and women older than 70 years.

Measurements: Information regarding demographic characteristics, comorbidities, mental status, dependency in activities of daily living, frailty, HRQoL, and other characteristics was obtained. Objective measurements of muscle strength and physical performance were grip strength using a manual hydraulic dynamometer and walking speed; measure of low muscle was by a calf circumference. Sarcopenia was defined according to the European Working Group on Sarcopenia in Older People criteria. Physical and mental components of the HRQoL were measured through the Medical Outcomes Study Health Survey Questionnaire 36-Item Short Form.

Results: Prevalence of sarcopenia was 20.6% for severe and 15.8% for moderate. After adjusting for sociodemographic and health characteristics, severe sarcopenia was significantly and inversely associated with both the physical (β = -5.39; P = .010) and the mental components (β = -3.69; P = .057) of HRQoL when compared with pre- and nonsarcopenic individuals.

Conclusions: Our results on the association between sarcopenia and HRQoL suggest that the latter declines in the presence of severe sarcopenia in older adults. This finding highlights the relevance of the early detection of sarcopenia in older individuals, and even that its detection must be a part of routine diagnosis procedures.

Frailty in community-dwelling older adults: association with adverse outcomes

Background: The study of frailty is important to identify the additional needs of medical long-term care and prevent adverse outcomes in community dwelling older adults. This study aimed to determine the prevalence of frailty and its association with adverse outcomes in community dwelling older adults.

Methods: A cross-sectional study was carried out from April to September 2014. The population sample was 1,252 older adults (≥60 years) who were beneficiaries of the Mexican Institute of Social Security (IMSS) in Mexico City. Data were derived from the database of the “Cohort of Obesity, Sarcopenia and Frailty of Older Mexican Adults” (COSFOMA). Operationalization of the phenotype of frailty was performed using the criteria of Fried et al (weight loss, self-report of exhaustion, low physical activity, slow gait, and weakness). Adverse outcomes studied were limitation in basic activities of daily living (ADLs), falls and admission to emergency services in the previous year, and low quality of life (WHOQOL-OLD).

Results: Frailty was identified in 20.6% (n=258), pre-frailty in 57.6% (n=721), and not frail in 21.8% (n=273). The association between frailty and limitations in ADL was odds ratio (OR) =2.3 (95% confidence interval [CI] 1.7–3.2) and adjusted OR =1.7 (95% CI 1.2–2.4); falls OR =1.6 (95% CI 1.2 2.1) and adjusted OR =1.4 (95% CI 1.0–1.9); admission to emergency services OR =1.9 (95% CI 1.1–3.1) and adjusted OR =1.9 (95% CI 1.1–3.4); low quality of life OR =3.4 (95% CI 2.6–4.6) and adjusted OR =2.1 (95% CI 1.5–2.9).

Conclusion: Approximately 2 out of 10 older adults demonstrate frailty. This is associated with limitations in ADL, falls, and admission to emergency rooms during the previous year as well as low quality of life.

Negative impact of oral health conditions on oral health related quality of life of community dwelling elders in Mexico city, a population based study

Aim: Oral health in old persons is frequently poor; non-functional prostheses are common and negatively affect quality of life. The objective of this study was to estimate the impact of oral health problems on oral health related quality of life in a sample of home dwelling Mexican elders.

Methods: Household survey in 655 persons 70 years old and over residing in one county in Mexico City.

Variables: Oral Health Related Quality of Life (Short version of the Oral Health Impact Profile validated in Mexico-OHIP-14-sp), self-perception of general and oral health, xerostomia, utilization of dental services, utilization and functionality of removable dental prostheses, dental and periodontal conditions, age, gender, marital status, schooling, depression, cognitive impairment and independence in activities of daily living (ADL). A negative binomial regression model was fitted.

Results: Mean age was 79.2 ± 7.1 years; 54.2% were women. Mean OHIP-14-Sp score was 6.8 ± 8.7, median was 4. The final model showed that men (RR = 1.30); persons with xerostomia (RR = 1.41); no utilization of removable prostheses (RR = 1.55); utilization of non-functional removable prostheses (RR = 1.69); fair self-perception of general health (RR = 1.34); equal (RR = 1.43) or worse (RR = 2.32) self-perception of oral health compared with persons of the same age; and being dependent for at least one ADL (RR = 1.71) increased the probability of higher scores of the OHIP-14-sp. Age, schooling, depression, cognitive impairment and periodontal conditions showed no association.

Conclusions: Oral rehabilitation can improve quality of life, health education and health promotion for the elder and their caregivers may reduce the risk of dental problems.

Exploring University Mexican Students’ Quality of Intra-Mathematical Connections When Solving Tasks About Derivative Concept.

The quality of the intra-mathematical connections made by Mexican university students when solving tasks on the derivative was characterized. The typology of mathematical connections and the quality levels of mathematical connections were used. Interviews were conducted based on eight tasks applied to three case studies. The results showed that the quality of the intra mathematical connections: different representations, procedural, implication, part-whole and meaning of the students, is found mostly at level 2 (consistent and argued mathematical connections) and only one student presented inconsistencies to find the correctness equation of the tangent line to the curve at a point (quality level 0 connection).

This article reports the findings of a survey on end-of-life (EOL) care in nursing homes of 18 long-term care experts across 15 countries. The experts were chosen as a convenience-based sample of known experts in each country. The survey was administered in 2016 and included both open-ended responses for defining hospice care, palliative care, and “end of life,” and a series of questions related to the following areas—attitudes toward EOL care, current practice and EOL interventions, structure of care, and routine barriers. Overall experts strongly agreed that hospice and palliative care should be available in long-term care facilities and that both are defined by holistic, interdisciplinary approaches using measures of comfort across domains. However, it appears the experts felt that in most countries the reality fell short of what they believed would be ideal care. As a result, experts call for increased training, communication, and access to specialized EOL services within the nursing home.

En el presente artículo se presenta una interpretación de la lectura agambeniana del asunto de la “vida privada” de Guy Debord tal como es considerada en el último volumen de la saga Homo Sacer a partir de su contextualización dentro de la obra tardía de Agamben. En primer lugar, se analiza la mención de “Guy” en Autorretrato en el estudio, la curiosa autoheterografía del filósofo publicada luego del abandono de la saga. Luego, a través de un recorrido por los tópicos debordianos que Agamben retomara en diversos escritos, y apelando a la noción de détournement así como a la valoración agambeniana de la comedia, se evalúa el semblante que el filósofo romano traza de su amigo en el contexto de sus reflexiones en torno a la forma-de-vida. De esta manera, se aborda la caracterización de Debord en el “Prólogo” de El uso de los cuerpos a partir de dos ejes que permitirían dimensionar su importancia para el desarrollo del libro: por un lado, la cuestión de la necesaria ridiculez de toda documentación de la vida privada y, por el otro, la forma de vida debordiana como una contribución fundamental a la definición de la escurridiza idea de forma-de-vida que, como dirá más adelante es “algo que no existe todavía en su plenitud y puede atestiguarse solo en lugares necesariamente no edificantes”.

This paper offers an interpretation of Agamben’s reading of Guy Debord’s “private life” within the framework of the last volume of the Homo Sacer saga, as well as Agamben›s recent work. In the first place, we analyze the mention of “Guy” in Self-portrait in the study, the strange autoheterography the philosopher published after he abandoned the saga. Secondly, we assess the biographical sketch/profile the Roman philosopher presents of his friend in the context of his reflections on the form-of-life through a review of the Debordian topics Agamben has taken up in many of his writings, and resorting to the notion of détournement as well as to the Agambenian considerations about comedy. In this way, the characterization of Debord is approached in the “Prologue” of The use of bodies from two axes that could allow to measure its importance for the development of that book: on the one hand, the question of the necessary ridiculousness of all documentation of private life and, on the other, the Debordian way of life as a fundamental contribution to the definition of the elusive idea of form-of-life that, as he will say later, is “something that does not yet exist in its fullness and can be witnessed only in places not necessarily edifying”.
